Once an iPhone is unlocked it should stay unlocked. I've never heard of an iPhone getting relocked. However, when you deleted all your information you probably set the phone back to factory settings which also meant your family member had to reactivate the phone with Apple. I suppose it is possible the Apple servers they accessed from Egypt did not recognize the phone as unlocked and actually did relocked it. That might also explain why your subsequent unlock request got the response you got.
